ALL incoming freshmen MUST have a complete physical and also complete the
health history questionnaire
prior to participating in any sports.

ALL 10^th, 11^th and 12^th grade student-athletes MUST pick up a health history questionnaire form from the high school between the hours of 7:30AM-3:30PM Mondays through Fridays prior to participating in any sports.
